Our social work services are currently contracted through LIV Health https://livhealth.org/
Their social workers conduct assessments when a patient is first admitted to hospice. They determine what needs the patient/family may have, then work to find ways to meet those needs. They may help apply for certain benefits, provide resources of available services and help them obtain those services. Patient advocacy is a big part of their roles. Our social workers are there to help our patients and families through transitions and hardships.
Their compassion and dedication ease the burdens felt by many.
