Patient Eligibility

A referral can be made by an individual, family member, caregiver, friend, community member, or health care provider.  Our interdisciplinary team consults with the individual’s primary health care provider and our medical director to ensure medical eligibility and to verify that the person will no longer benefit from curative treatments or does not wish to pursue further curative treatments.

Experts agree that individuals need to be enrolled in hospice care for at least 60 days or longer to maximize our benefits of pain and symptom management, as well as spiritual and bereavement support services.

One of the most common sentiments from families who’ve been helped by hospice care is, “we wish we had known about Hospice sooner.”  The majority of Americans say they want excellent pain management, comfort, and support for the patient and the family at the end of life.
